We accept insurance, self-pay and also offer sliding scale options for payment.
Our practice is in-network with the following insurance companies:

United Healthcare/Optum PPO
Medicare
HSA accepted
Most insurance policies cover a large portion of testing.
Insurance benefits will be verified and confirmed with client before services are provided. Please keep in mind this is an estimate, and all insurance companies differ in terms of coverage and verification.
Like all medical providers, portions of the bill not covered by insurance will be billed directly to the patient.
